In order to obtain a "No Fee" Cook County Sticker, you must visit your local Township or submit your application by mail to Cook County Dept of Revenue, PO Box 4523, Carol Stream, IL 60197-4523. If you would like an application form and have not received one by mail, please visit your local township or contact the Cook County Department of Revenue at (312)603-6961 to have one sent to you.

"No Fee" Sticker Categories are as follows:

PH – Physically handicapped
DV – Disabled Veterans
Z – School, Church & nonprofit vehicles
M – Municipal Owned
SR – Seniors
